Tax classification

Complete this section by selecting whether an individual or a business receives the income. Most LLC’s will report as individuals, unless you’ve specifically indicated to the IRS that your LLC is an S Corp, C Corp, or partnership.

  • “Individual” if you’re an individual or sole proprietor
  • “Individual” if you’re a single-member LLC where the owner is an individual — this accounts for most LLC’s
  • "Business" if you’re an S Corp, C Corp, partnership, trust/estate, or if you’re an LLC that has filed a Form 8832 or a Form 2553 to be taxed as a corporation or designated as a partnership

Next, indicate your status in the US for tax purposes.

  • “Yes” if you are a US citizen, US permanent resident (green card holder), or other US resident alien
  • “No” if you aren’t
  • “I’m not sure if I’m a US resident alien” if you’re unsure and want to complete a quick Substantial Presence Test to determine your tax residency status

Basic information

  • Input your name exactly as it appears in any of your correspondence with the IRS
  • Select your US Tax Identification Number (TIN) type
    • For “Individuals” (including single-member LLC’s), enter your SSN in the US Tax Identification Number (TIN) value field
    • If you selected anything other than “Individual,” your US Tax Identification Number (TIN) type is an Employer Identification Number (EIN).
  • Input or edit your address information
